how to prepare

Don't worry, there's no dress code, Just come as you are. However, completed paperwork is required prior to our first meeting. Please plan to arrive at least 15 minutes early to complete the entire packet- additional time if you think you need it so as to not cut into our time together. You may request it be sent via email or arrange a time to pick up the paperwork at my office if you prefer to complete it at home and bring it with you. A separate completed packet is required for EACH person attending the session, including spouses and minors. If you are a parent or guardian bringing a minor you have custodial custody for, a copy for my records of the complete agreement including all addendums must be provided. I am not in-network on any insurance companies, but am happy to code your receipt accordingly for you to submit for reimbursement. Please arrive prepared to make payment at the beginning of each session.
